Since 1975, Pathology Services Laboratory, P.A. has provided the Arkansas River Valley and surrounding areas with outstanding diagnostic services.
Our highly trained pathologists are fellows of the College of American Pathologists and are Board Certified by the American Board of Pathology in Anatomic and Clinical Pathology. Utilizing the most technologically advanced diagnostic systems, our pathologists analyze thousands of specimens a year delivering clear, concise, and accurate diagnostic reports in a timely manner.

Surgical Pathology
Our board-certified pathologists and licensed histotechnologists have extensive experience in a wide range of specialties including:

To uphold a high standard of care, our CLIA certified laboratory offers innovative technology including the Tissue-Tek VIP® 6 AI Tissue Processor with advanced infiltration methods to maintain the highest level of tissue quality and integrity, automated cassette printing with barcode labeling for accurate specimen identification, and automated Benchmark staining systems with a comprehensive up to date menu of immunohistochemical markers and special stains for greater diagnostic accuracy.
Gynecologic Cytology
Our gynecologic cytology service provides screening of specimens by our licensed cytotechnologists and utilizes the ThinPrep® line of technology to provide incredibly accurate interpretations by our cytopathology team.
- The ThinPrep® Genesis™ Processor uses a chain of custody verification system, which prevents sample mix-ups for confidence in your results.
- The ThinPrep® filter technology optimizes cellular presentation and diagnosis.
- The Cytyc ThinPrep ® Imaging System uses computer-assisted screening to enhance the quality of diagnostic interpretations.
Plus, our Hologic Aptima® high-risk HPV and HPV 16, 18/45 genotype assays assists in early disease detection and risk stratification.
Non-Gynecologic Cytopathology
With board certified cytopathologists on staff and extensive experience in non-gynecologic testing, we provide definitive diagnostic interpretations using the most up to date clinical practice guidelines.
Our licensed cytotechnologists screen all non-gynecologic cytopathology specimens and are available to assist with Fine Needle Aspiration (FNA) on-site adequacy evaluation.
Molecular Infectious Disease Testing
To deliver comprehensive and accurate molecular infectious disease testing, we utilize nucleic acid amplification testing (NAAT) with the Hologic Panther® Aptima® Assays. We provide testing for:
- Chlamydia trachomatis
- Neisseria gonorrhoeae
- Trichomonas vaginalis
- Candida species group
- Candida glabrata
- Bacterial vaginosis
- Herpes Simplex Virus 1 & 2
- High-risk HPV
- HPV 16, 18/45 genotype
Laboratory Medical Services
Board-certified clinical pathologists with extensive experience currently serve as medical directors for high-complexity laboratories and are available for clinical consultation.
Connectivity Solutions
A wide range of test ordering and result retrieval methods, including fax, on-line physician portal, courier service and EMR interface, are available to accommodate individual physician or clinic preferences.
Our physician portal provides access to your patients’ reports with real-time report status 24/7 from any computer with internet access. E-requisitions can be used for ordering lab tests, or paper requisitions are also available.
Easily Accessible Staff and Pathologists
Direct phone access to laboratory staff and pathologists is available to all referring providers and staff to quickly answer any questions, provide guidance on specimen collection and storage, discuss diagnostic interpretations, and assist with ordering of ancillary tests. You never have to worry about talking to an automated answering system.
Dependable Courier Services
Our in-house couriers, who are trained in specimen identification and tracking, provide dependable transport of specimens from referring provider offices and hospitals to our laboratory, ensuring timely and accurate delivery of specimens.
Friendly Billing Services
Our experienced billing professionals are happy to work directly with patients who have billing concerns or any referring provider’s staff to resolve any billing issues or questions. Pathology Services Laboratory is a participating provider with all major health insurance plans, and we offer payment arrangements to accommodate uninsured or under-insured patients.

Bills from Pathology Services Laboratory are for the evaluation of tissue, blood or cellular material submitted by your physician to us. Pathology is a subspecialty of medicine in which licensed physicians study submitted specimens to evaluate for disease processes and report the findings and diagnosis to your physician. These services are performed in our laboratory and are separate from the collection of the specimen, which was performed by your physician at their office or hospital. We abide by the federal regulations contained in the No Surprises Act. Our Notice Regarding Patient Protections Against Surprise Billing can be found in the Resources Section.
You can obtain a copy of your report from your physician’s office or hospital where your procedure was performed. You can also contact our medical records department at 479-968-6781 or 1-800-874-4904.
Payments are accepted by mail, phone, or online. We accept checks and major credit cards. The Billing Department is available Monday-Friday, 8 a.m.-5 p.m at 479-968-6781 or 1-800-874-4904.

Specimen collection and storage guidelines for each test type can be found under the Resources Tab Specimen Collection and Storage Guidelines.
- Complete the Test Requisition and attach the patient’s billing information. Test requisitions can be found in the Test Requisitions section under the Resources Tab. Requisitions are also available by calling our office at 479-968-6781 or 1-800-874-4904 or emailing PSLab@pathologyserviceslab.com.
- Place the appropriately labeled specimen in a biohazard bag with the test requisition and billing information in the outside pocket. Please see additional FAQs for instructions on how to label the specimen.
- If you are an existing client, place the specimen in the Pathology Box at your facility. If your facility is not scheduled for daily pick-up, please also call Pathology Services at 479-968-6781 or 1-800-874-4904 and request pick-up.
- If you are not an existing client, please call Pathology Services at 479-968-6781 or 1-800-874-4904. We will assist you with specimen preparation, answer any questions that you may have, and arrange for pick-up of the specimen.
The test requisition must be accurately and legibly filled out under supervision of the ordering physician. The following information is required:
- Patient’s full name. Note of any name change in the last five years is also helpful for retrieving previous patient records.
- Patient’s date of birth.
- Patient’s medical record number.
- Source and type of specimen. Properly identify the specimen by listing WHAT the specimen is (mass, tumor, bone, etc.) and WHERE it was obtained (anatomical site including LEFT or RIGHT if applicable).
- Ordering clinician and facility.
- Date and time of specimen collection.
- Clinical history relating to the submitted specimen (Pre-operative and Post-operative diagnoses).
- For breast biopsy or resection specimens, the time of removal of the tissue and the time of tissue immersion in fixative.
- For gynecologic cytology, Menstrual status (LMP, hysterectomy, pregnant, postpartum, hormone therapy) and whether the patient had a previous abnormal cervical cytology result or previous treatment, biopsy, or surgical procedure.
- For fetus or products of conception greater than 12 weeks, Consent for Final Disposition should be attached to the Test Requisition.
- Patient’s demographic and insurance information.
Please attach the patient’s demographic/insurance sheet and completed Advanced Beneficiary Notice, if applicable to the requisition.
- Immediately after sample collection and if possible, in front of the patient, the specimen vial, specimen container, or glass slide must be labeled with the patient’s full name and one additional unique patient identifier (either date of birth or medical record number) matching the identifiers on the test requisition.
- The specimen label must be affixed to the vial, cup, or container, not the lid or cap.
- If multiple specimens are to be examined and diagnosed individually, each specimen must be submitted in a separate container, slide, or vial labeled with the two unique patient identifiers and the specimen source.
- For glass slides the required information must be written in solvent resistant pen or pencil on the frosted end of the slide.
To ensure that claims are properly submitted to the appropriate payer, please provide the following information:
- Patient’s Full Name, Date of Birth, SSN
- Patient’s Complete Address
- Patient’s Phone Number
- Copy (front and back) of each of the patient’s insurance cards
- Ordering Physician’s Full Name, NPI, phone number, and address
- Appropriate ICD-10 diagnosis code for each ordered test or specimen submitted.
- A completed Advance Beneficiary Notice (ABN) form signed by the patient if the test or service is expected to be denied by Medicare. The ABN form can be downloaded from this website on the Resources tab under Billing Documents.
Please advise patients that they may receive a bill for laboratory services from Pathology Services Laboratory, and that they are responsible for annual deductibles, co-payments, and any balance not covered by their insurance plan. Yes, to ensure positive patient identification and optimum diagnostic interpretation, certain specimens cannot be accepted.
Specimens will be rejected under the following conditions:
- The incorrect specimen type is submitted for the requested test.
- The specimen exceeds the acceptable age limit for the requested test.
- The specimen container and/or test requisition is/are not labeled with two unique patient identifiers.
- The two unique patient identifiers on the specimen container do not match those on the requisition or electronic order.
- The specimen source is not listed on each specimen container when multiple specimens are submitted on one patient.
- The specimen is submitted in an unacceptable container type, such as glass or container without the proper fixative.
- The specimen has a needle attached.

Yes, we accept specimens Monday through Friday from 8 am until 5 pm at 1430 West C Street, Russellville, AR.
Call our office at 479-968-6781 or 1-800-874-4904 between 8 am and 5 pm, Monday through Friday. We will put you in contact with the pathologist on your case or the on-call pathologist. If you need to speak to a pathologist outside regular business hours, please refer to the on-call schedule with phone list that was provided to your facility.
Pathology reports can be transmitted to you via fax, physician/practice portal, courier service, and/or EMR interface.
